Geriatric care certification achieved
Althea Denham RN, CCM, GCM with Home Healthcare, Hospice and Community Services has achieved certification as a geriatric care manager. The designation involves a rigorous exam and two years of work in the field of case management with older adults. Knowledge of health care and family systems and interpersonal communications and community resources is necessary.
As a geriatric care manager with HCS, Denham evaluates all aspects of an older adult’s personal situation, makes recommendations, and arranges services that can assist senior citizens to maintain independence and live safely at home. She also acts as a liaison with family members, particularly those at a distance, and physicians as needed. Denham has been a nurse with Home Healthcare, Hospice and Community services for 10 years in both the Keene and Peterborough HCS offices. She is a member of the National Association of Professional Geriatric Care Managers, an organization of practitioners whose goal is the advancement of expert assistance to older adults and their families.
