Monadnock Community Hospital will host a free seminar, “Care At Home: Understanding Home Healthcare, Hospice & Community Services,” on Wednesday, March 25, at 5:30 p.m. in the Bond Wellness Center Conference Room in Peterborough.
The session is designed for older adults, caregivers and adult children supporting aging parents who want to understand care options before a health crisis occurs. Topics include visiting nurse services, home health care, caregiver and family support, community programs, and when hospice or palliative care may be helpful.
The program will also explain how HCS collaborates with Monadnock Community Hospital to coordinate care across home, outpatient and hospital settings.
